Complex cases & hospital discharge
Maridi Care can consider complex referrals, including people who are homeless, being discharged from hospital, or requiring enhanced support due to mental health, autism, behavioural or social care needs.
How we assess
Higher-support referrals get more scrutiny, not less. The steps below sit alongside our standard referral pathway.
We review assessments, risk history, care plans, safeguarding information and compatibility with the shared living environment before confirming suitability.
Where additional needs are identified, we provide a tailored support proposal covering staffing, support hours, risk controls, review arrangements and pricing.
Medication administration, personal care or clinical input can be arranged through suitable partner providers where required and agreed by the commissioner.
